ENROLMENTS FOR THE THIRD ROUND OF VACCINATION FOR PERSONS CONDUCTING CLASSES

Enrollment in the third round of vaccination against COVID-19 for persons conducting classes with students has begun.

FOR WHO?

  1. academic teachers,
  2. doctoral students conducting classes with students,
  3. persons conducting classes with students on the basis of civil law contracts, who have not registered in previous rounds or have registered for vaccination, but for random reasons could not (can not) appear for vaccination within the specified time.

HOW TO ENROLL?

Those interested in vaccination against covid-19 are asked to register for vaccination from 8 April 2021 until 9 April 2021 at 9:00 a.m., using the form available at: https://uni.wroc.pl/covid-nauczyciele-akademiccy/ .

 Vaccination against COVID-19 will be carried out on 11 April 2021 by:

Attention! It is not possible to change the date of vaccination once set or to change the place of vaccination.

To confirm your eligibility for COVID-19 vaccination, you will receive a referral, which you will find on the Individual Patient Account available on the website www.pacjent.gov.pl. Vaccinations will be carried out in alphabetical order of the persons qualified, and information on the date and time of vaccination (first and second dose) will be transmitted via SMS and e-mail to the phone number and e-mail address specified in the registration form.

For vaccination against covid-19, you must always present your identity card and a printed and completed (in terms of personal data and preliminary questions) form, which is Annex no. 1 to the communication below, together with information on how to prepare for vaccination.

An obligation has been introduced to inform the COVID-19 coordinator – Director-General of the fact of vaccination refusal to the following e-mail address: dyrektor.generalny@uwr.edu.pl.
